The Role
We are looking for an experienced Integrations Engineer with strong specialisation in HL7 (v2.x) and FHIR, ideally with hands-on NHS integration experience.
You will design, build, and support integrations between xWave Refer and hospital systems, including RIS, LIMS, PAS, and EPR platforms.
This role sits at the heart of our growth. Every new Trust deployment depends on clean, resilient, secure integrations - and you will be central to making that happen.
You’ll work directly with NHS Trust integration teams, interface engineers, and IT departments, representing xWave in live clinical environments.
Core Responsibilities
Design and implement HL7 v2 and FHIR interfaces, including ADT, ORM, ORU, and SIU message types
Build message transformations, routing logic, and validation
Manage ACK/NACK workflows and reconciliation processes, helping define workflows in advance of project kickoff.
Troubleshoot production interface issues at both the message and transport levels
Develop and support FHIR-based integrations (R4 preferred)
Integrate with NHS Spine services and PDS APIs where required
Work within NHS network environments, including HSCN and site-to-site VPNs
Configure and support secure connectivity, including IP whitelisting, certificate exchange, and TLS encryption
Collaborate with Trust IT and integration teams during design, testing, and go-live
Improve onboarding processes and develop repeatable integration deployment patterns
Maintain clear technical documentation and interface specifications
Required Experience
4+ years working with HL7 v2.x
Strong understanding of ADT, ORM, and ORU message structures
Experience working in the NHS or UK healthcare environments
Hands-on experience configuring or supporting VPN connections for healthcare system integrations
Experience integrating with RIS, LIMS, PAS, or EPR systems
Strong understanding of patient identity management, including NHS number usage and MRN mapping
Ability to troubleshoot across the application, network, and transport layers
A willingness to learn and get involved in other parts of the business, such as engineering, SRE, product, and growth
Highly Desirable
Experience integrating with Epic Systems, MEDITECH, and Oracle Health.
Experience using integration engines such as Mirth, Rhapsody, or Cloverleaf
Experience with FHIR R4 APIs
Exposure to NHS Spine or PDS services
Experience in radiology or pathology workflows
AWS infrastructure exposure
Experience with Claude Code
Experience in SRE environments
